What is Case Management?

Case Management assists individuals determined eligible for service from the Division of Developmental Disabilities in accessing comprehensive medical, social, educational and other specialized services. Case Managers are professionals who are trained in the field of mental health and/or closely related fields. Each individual who applies for services from the Division of Developmental Disabilities is assigned a case manager who will assist the person and his or her family to identify, locate, coordinate and monitor services that meet the individual’s distinct needs.

For many Missourians case management is only available from the state through Regional Offices. However, in a few counties, that service is also available locally from the county Senate Bill 40 Board. In Cooper County, you are very fortunate to have a local case management provider.

Eligibility Requirements

To be eligible for services from CCBSS or CMRO, an individual must have a developmental disability that:

  • Began before the age of twenty-two.
  • Is likely to continue indefinitely.
  • Results in problems with two or more of the following:

    -living and working independently
    -written or verbal communication
    -self direction
    -learning
    -mobility
    -self-care

Cooper County Residents who have developmental disabilities such as autism, cerebral palsy, Down Syndrome, epilepsy, congenital disabilities, severe learning disabilities and mental retardation may be eligible for services.

What is the Cooper County Board of Sheltered Services?

CCBSS is a local governmental entity comprised of 9 men and women, (3 of whom have a family member with a developmental disability), which is responsible for administering county funds for the development and operation of services for individuals with developmental disabilities in Cooper County.

  • Time spent with you developing a working plan to address the identified supports needed.
  • An active role in all aspects of services you receive.
  • Assistance in finding resources within the community.
  • Referrals to community resources such as transportation, counseling, recreational groups, etc.
  • Monitoring of services that you receive; to guarantee quality and the proper delivery.
  • Assistance in completing paperwork.
  • Creativity and flexibility of time and resources offered.
  • Information in a timely manner.
