What Case Management Offers You

  • Time spent with you developing a working plan to address the identified supports needed.
  • An active role in all aspects of services you receive.
  • Assistance in finding resources within the community.
  • Referrals to community resources such as transportation, counseling, recreational groups, etc.
  • Monitoring of services that you receive; to guarantee quality and the proper delivery.
  • Assistance in completing paperwork.
  • Creativity and flexibility of time and resources offered.
  • Information in a timely manner.
  • Accountability of services authorized and money allocated.

What is Case Management?

Case managers assist individuals determined eligible for services from the Missouri Division of Mental Retardation and Developmental Disabilities in accessing comprehensive medical, social, educational and other specialized services. Case managers are professionals who are trained in the field of mental health and/or closely related fields. Each individual who applies for services from the Division of MRDD is assigned a case manager who will assist the person and his or her family to identify, locate, coordinate and monitor services that meet the individual’s distinct needs.

Eligibility Requirements

To be eligible for services from CCBSS or CMRC an individual must have a developmental disability that:

  • Began before the age of twenty-two.
  • Is likely to continue indefinitely.
  • Results in problems with two or more of the following: Living and working independently, written or verbal communication, self direction, learning, mobility, self-care.

Cooper County residents who have developmental disabilities such as autism, cerebral palsy, Down syndrome, epilepsy, congenital disabilities, severe learning disabilities and mental retardation may be eligible for services.

A case manager (whose office is at 1620 West Ashley Road in Boonville) will be assigned to call you, help you complete the paperwork to determine if you are eligible for targeted case management services, and help you access the services you need.
